Position Summary

Title: Coordinator of Windows Memory Support Program

Reports to: Executive Director, Executive Director of Meadows, DOW, and Community Life Coordinator (varies based on the facility)

Position Summary: Implements a program of activities and cognitive support that promotes social connectedness and a sense of well-being and personal satisfaction among residents who are struggling with mild cognitive impairment or psycho-emotional adjustment to chronic illness. Follows the overall Activities philosophy of addressing resident’s social, emotional, physical, spiritual, and vocational health.

Routine Function of the Job:

Builds and maintains a trusting relationship with each resident assigned to the Windows Memory Support program to encourage participation.

• Keeps family members, caregivers, DOW, and executive director informed about resident involvement in the program. Maintains a family call log.

• Collaborates with Community Life Coordinator and plans a resident focused daily activities schedule designed to stimulate interest and to support resident success offering a variety of events including small group as well as individualized sessions.

o Creates monthly calendars of Windows activities, combining as appropriate with the facility wide calendar, and distributes to established list of recipients.

o Creates a resident specific weekly calendar of activities, appointments, which is consistent with the customer service plan sharing with resident and family, if desired.

• Leads activities creating opportunities for resident’s to experience the satisfaction of involvement and achievement while minimizing the effects of personal limitations.

• Consistently observes and evaluat4es resident responses to activities, daily routines, unusual events, etc. to determine whether they build resident well-being and peace of mind.

• Provides the residents with cues and reminders about activities and programs throughout the day.

• Provides modeling and suggestions to co-workers related to communication and behavioral interventions that are individualized to the resident’s needs.

• Collaborates with the facility team to identify those residents who demonstrate behavior consistent with criteria for Windows Memory Support participation.

o Helps to identify those participants whose needs exceed that provided by the program.

• Evaluates the results of existing activities in order to determine what, if any, changes are needed to improve the program’s effectiveness.

o Maintains daily attendance records and other data to assess program quality and effectiveness.

  1. Works with other Community Life members (both AL and Meadows), PCAs, and other departments to continually enhance the Windows Memory Support program.
  2. Ensures availability of a selection of activity centers, and activities that are meaningful, enjoyable, and individually done with a minimum of frustration.
  3. Assists in co9nducting initial and bi-annual cognitive/functional tests or assessments on residents.
  4. Communicates and promotes activities and events in order to encourage maximum resident and co-worker participation.
  5. Participates in resident assessment planning meetings and implements those goals and suggestions specifically related to the Memory Support goals.
  6. Offers routine brain fitness computer programs) Posit Science) to all residents and is a spokesperson/coach for promoting and implementing these courses.
  7. Maintains and protects resident and co-worker confidentiality.
  8. Recognizes and respects residents’ rights.
  9. Recognizes and reports fire/safety hazards in building. Reports maintenance needs promptly. Takes all appropriate safety precautions to minimize risk of injury to resident, self and other co-workers.

Management/Supervision

  1. Assist in the orientation and training of departmental personnel to insure that they understand the Windows Memory Support program, philosophy, and guidelines.
  2. Provide modeling and suggestions to the PCAs and volunteers as they interact with residents and implement the program of activities.

  5. Ability to be self-supervising and self-motivating: must be able to set priorities and follow through on same.
  6. Organizational skills, including the ability to handle multiple demands/tasks concurrently. Must be adaptable and flexible enough to adjust to meet resident needs.
  7. Motivational skills, including the ability to positively influence residents.

Work Environment

  1. Requires standing, walking, twisting, bending, pushing, kneeling, stooping, stretching and lifting a minimum of 25 pounds.
  2. Requires normal range of hearing and vision, hand-eye coordination and manual dexterity.
  3. Must be capable of operating: passenger vehicles, 14 passenger vans, VCR, TV, office equipment, fire and safety equipment, gardening tools.
  4. Workplace includes the entire building and surrounding grounds, spas and swimming pools, all areas necessitated by off-site field trips and events.

Minimum Qualifications:

  1. Experience: Prior experience in group socialization or recreation preferred especially related to older population
  2. Education: High School diploma or GED
  3. Certification/License: Valid driver’s license – must have clean driving record with no major violation over the past five years
  4. Computer skills for word processing, calendars, and email preferred.
