Title : Assistant Director of WellnessReports to : Director of WellnessSupervises: Nurses and Personal Care StaffPosition Summary : Provides hands-on supervision of personal care staff, ensuring provision of quality resident care and the promotion of resident wellness. Provides direct nursing services to the residents ensuring optimal health and safety. In conjunction with the Director of Wellness, supports all policies, programs and philosophies of Country Meadows. Other relevant duties as may be required or assigned by management.Routine Functions of the Position :
Meet with each resident on a regular basis to provide health and wellness checks and answer any health related questions. Respond appropriately to resident illness or injury. Facilitate scheduling of and transportation to physician office visits for residents. Assure that medical orders are implemented. Collaborate with other wellness team members to continually monitor resident service needs, establish service plans and assist in identifying appropriate resident placement within Country Meadows continuum of care. Communicate information from the customer service plan to the personal care staff and ensure that staff provides services specified in the plan. Order medications and oversee that medication schedule is maintained for residents requiring medication assistance. Follow policies and procedures for medication system integrity. Ensure completion of any medication related documentation. Supervise and direct personal care staff and nurses under the direction of the Director of Wellness. Participate in coworker evaluations and coaching. Maintain confidentiality of co-worker information. Maintain open communication with residents, physicians and responsible parties to ensure optimal provision of care to the residents. Protect resident rights and confidentiality of information according to state and federal laws. Actively protect resident and co-worker safety by recognizing and reporting or taking immediate action to minimize risk of accident or injury. Appropriately handle on-the-job injuries reported by co-workers. Respond to resident emergencies and react appropriately. Implement action plan to decrease risk of further incident occurrence. Follow requirements for completion of any safety related reports. Participate in the recruitment, hiring, training and on-going education of new coworkers. Critical success factors:

Work environment:
Ability to stand, sit, walk, kneel, reach, stretch, carry up to 15 lbs., and to support up to 70 lbs. Secure proper assistance for transferring and lifting residents as needed. Minimum qualifications:
Minimum of one year experience in assisted living or other health care. Prior supervisory experience in the healthcare industry preferred but not required. |
